Your First Visit
For your first visit, please arrive 15 minutes early to fill out new patient paperwork and bring the following items:
- Insurance card
- Co-pay if required by your insurance (we accept cash, check and Mastercard/Visa)
- Referral from your primary care physician (if needed)
- Any urologic x-rays (including IVP, CT scan, ultrasound, etc.)
- List of prescriptions and medical conditions

Insurance
The list below may not be conclusive at all times. Patients must call their insurance company to determine their policy coverage and requirements. We accept the following insurance plans:
- Aetna US Healthcare (referral needed except for PPO)
- BCBS of Maryland (also out of state BCBS)
- Blue Choice (paper referral needed)
- BSPOS (paper referral needed)
- Cigna (script accepted for Managed Care)
- Coventry (check with individual plan to determine if referral is needed)
- EHP (paper referral needed)
- Fidelity (no referral needed)
